Depression Not Improving Despite Doing Everything

Answered 14 October 2026 at 6:36 pm PKT

It is exhausting to keep doing every suggested exercise and still wake up feeling the same heavy emptiness. This page speaks to that hopeless moment and explores why improvement can stall and what you can still try.

I've been following all the advice and doing the things you suggested, but I still feel just as sad and empty as before. Nothing seems to make any difference, and I'm starting to lose hope that anything will ever help.

That is genuinely hard, and it makes complete sense that you are losing hope. When you do everything you are told and still feel the same, it feels like the world is lying to you about this getting better. Please know – no, I won’t say that. Let me just say: you are not doing anything wrong. Depression is stubborn like that. It does not respond to a single round of effort, and it often gets worse before the small shifts start to show. The fact that you are still reading this, still trying, means something important is still there.

Don’t keep doing the same thing harder

If the routine you built – walks, journaling, breathing, whatever you tried – is not shifting the emptiness, then the next step is not to push more. It is to stop treating the sadness as a problem to solve alone. The heaviness may need a different kind of help: a professional who can sit with you, face to face, and look at what is underneath. Many people try self-help for months only to feel worse, and that is exactly when a therapist or a psychiatrist becomes the right move, not a failure.

Would you consider doing just one thing? Go to a general practitioner – any clinic – and ask for a basic blood test: vitamin D, B12, and thyroid. These three are very common in Pakistan and can drain every bit of your energy and mood. If that comes back normal, then the next step is a psychiatrist who can prescribe something that works differently than just talk or habits. There is nothing weak about that. It is the most practical thing you can do right now.

And if that feels too big, then for today, do one tiny act of care that has no pressure to fix anything: sit with a cup of chai and let yourself do nothing, no checklist, no goal. That is enough. Zara is here to talk if you want to say more.
